You will choose one of our Network Primary Care Dentists for you and your covered dependents, selecting from a wide network of private dental offices. Included with this brochure is a listing of Primary Care Dentists who have contracted with LIBERTY Dental Plan to provide dental care. Please note that a primary care dentist must be selected for you and your covered dependents. If a primary care dentist is not selected, you and your covered dependents will be assigned to a primary care dentist located close to your home address.
