You will choose a contracted Primary Care Dentist for you and your family. To obtain services, contact your assigned dental office to schedule an appointment. If you require a Specialist, LIBERTY Dental Plan will arrange for you to receive services from a contracted Dental Specialist if the necessary treatment is outside the scope of General Dentistry.
